This policy covers the vitalitymed.care website. We collect the contact details you choose to send us, use them to respond to you, and don't sell them. The website isn't used to collect patient health information. Health information we handle while caring for patients is protected under HIPAA and governed by the referring practice's Notice of Privacy Practices.

Scope
This Privacy Policy explains how VitalityMed Care ("VitalityMed," "we," "us") collects, uses and shares information through vitalitymed.care and related pages (the "Site"). It applies only to the Site. It does not apply to other websites we link to.
Patient health information
The Site is not used to collect patient health information, and we ask visitors not to include it in our forms or in email.
When we provide chronic care management, remote patient monitoring or transitional care services, we do so on behalf of the patient's physician practice or health system. In that role we handle protected health information as a business associate under the Health Insurance Portability and Accountability Act (HIPAA). That information is protected under HIPAA and our agreement with the practice, and its use is described in the practice's Notice of Privacy Practices, not in this policy.

Information we collect
Information you give us
When you submit our contact form, we collect your name, work email, phone number, practice or organization, role, specialty, number of providers and any message you write. When you call or email us, we collect the information you choose to share.
Information collected automatically
Like most websites, the Site's hosting and security services process technical information such as your IP address, browser type, device information, the pages you request and the date and time of your visit. This is used to deliver the Site, keep it secure and protect our forms from spam and abuse.
Cookies and similar technologies
We do not use advertising or cross-site tracking cookies. Our security provider may use strictly necessary cookies or similar technologies to protect the Site and to verify that form submissions come from a person rather than an automated program. If we use website analytics, we use a privacy-focused service that reports aggregate traffic without advertising cookies.
Fonts
The Site loads fonts from Google Fonts. To deliver them, your browser connects to Google's servers, which receive your IP address. Google's handling of that information is described in Google's privacy policy.
How we use information

We do not use the information collected through the Site for targeted advertising.
How we share information

- Service providers who host and secure the Site, protect our forms from spam and deliver form submissions to our team by email. They may use the information only to provide those services to us.
- Legal requirements, when required by law, subpoena or other legal process, or to protect the rights, safety and property of VitalityMed, our clients, patients or others.
- Business transfers, as part of a merger, acquisition, financing or sale of assets, subject to this policy.
- With your consent or at your direction.
How long we keep information
We keep contact form submissions and related correspondence for as long as needed to respond to you and manage our business relationship, and as required by law. We then delete or de-identify them.
Security
We use administrative, technical and physical safeguards appropriate to the information we handle, including encryption in transit for the Site and form submissions. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
Your choices and rights
You can ask us to access, correct or delete the personal information you've submitted through the Site, or ask us to stop contacting you, by emailing care@vitalitymed.care or calling (800) 570-8160. We may need to verify your identity before acting on a request.
California residents
If the California Consumer Privacy Act applies, California residents have the right to know what personal information we collect and how we use and disclose it, to request deletion or correction, and to opt out of the sale or sharing of personal information. We do not sell or share personal information as those terms are defined in that law. You may use an authorized agent to submit a request, and we will not discriminate against you for exercising your rights.
Children
The Site is intended for health care professionals, patients and their families. It is not directed to children under 16, and we do not knowingly collect personal information from them through the Site.
Links to other websites
The Site links to research articles and other websites we don't control. Their privacy practices are governed by their own policies.
Changes to this policy
We may update this policy from time to time. The effective date at the top of this page shows when it was last changed. Material changes will be posted on this page.
