This website describes VitalityMed's programs for practices and patients. It is not medical advice and does not create a patient-provider relationship. In an emergency, call 911: home readings are reviewed daily, not monitored live.

Acceptance of these terms

These Terms of Use govern your use of vitalitymed.care and related pages (the "Site"), operated by VitalityMed Care ("VitalityMed," "we," "us"). By using the Site, you agree to these terms and to our Privacy Policy. If you don't agree, please don't use the Site.

Not medical advice

Content on the Site is for general information about our services. It is not medical advice, diagnosis or treatment, and it is not a substitute for care from a qualified clinician. Using the Site does not create a patient-provider relationship with VitalityMed or any clinician.

If you have a medical emergency, call 911. Readings from home devices in our programs are reviewed daily by nurses and are not monitored live. Patients enrolled in our programs should follow their doctor's instructions and contact their doctor's office or 911 as appropriate.

Research and program information

The Site cites published research and public health data, with links to the original sources. Published results reflect the specific programs and populations studied and are not a promise of any particular outcome. Program eligibility and participation are determined by the referring practice and applicable program requirements. Information about quality measures and program requirements is general and is not legal, billing or compliance advice.

Using the Site

You may use the Site for lawful purposes. You agree not to:

  • Submit patient health information or other sensitive information through the Site's forms
  • Submit false or misleading information or impersonate another person
  • Interfere with the Site's operation or security, or attempt to gain unauthorized access to it
  • Use the Site to send spam or unlawful content
  • Use automated tools in a way that places an unreasonable load on the Site

Intellectual property

The Site and its content, including text, graphics, logos and design, are owned by VitalityMed or its licensors and protected by intellectual property laws. You may view and print pages for your personal or internal business use. You may not copy, modify or distribute Site content for commercial purposes without our written permission. "VitalityMed" and the VitalityMed logo are our trademarks.

The Site links to websites operated by others, including research publishers and public health agencies. We don't control and aren't responsible for their content or practices.

Disclaimers

The Site is provided "as is" and "as available." To the fullest extent permitted by law, we disclaim all warranties, express or implied, including warranties of merchantability, fitness for a particular purpose and non-infringement. We work to keep the Site accurate and available but do not guarantee that it will be error-free, current or uninterrupted.

Limitation of liability

To the fullest extent permitted by law, VitalityMed will not be liable for any indirect, incidental, special, consequential or punitive damages arising from your use of, or inability to use, the Site. This section does not limit liability that cannot be limited under applicable law, and it does not govern services provided under a separate agreement with a practice or health system.

Governing law

These terms are governed by the laws of the State of California, without regard to its conflict-of-law rules.

Changes to these terms

We may update these terms from time to time. The effective date at the top of this page shows when they were last changed. Continued use of the Site after changes are posted means you accept the updated terms.
